Bluebird Care Richmond & Twickenham has introduced its Care Development Lead, Sylwia Sobolewska, who helps residents and their families navigate their care options.

Sylwia provides information, guidance and reassurance to people considering care, helping them make informed decisions about the support they need.

Before care begins, she meets with customers and their families to understand their individual needs, preferences and priorities.

She then works closely with the care team to ensure appropriate support is put in place.

A key part of her role involves building relationships with local healthcare professionals, hospitals, care homes and other organisations to help identify how Bluebird Care can support people in the community.

Sylwia also explores local services and resources to ensure customers can access appropriate support.

When required, she liaises with hospital teams and other professionals to help people transition from hospital or a care home back to their own homes as smoothly as possible.

Her role focuses on helping people receive care in familiar surroundings while providing ongoing support to both customers and their families.
